Sports Physical and Parent Consent Forms
A student will need a Sports physical dated after April 15, 2024 to try out for a 2024/2025 team. If it is dated before, per TSSAA policy, your student cannot participate. Below are the TSSAA required forms:

Printed forms are available in the front office and the cafeteria. **Once completed, students will need to turn them in to the front office; this will be the only location to turn in the forms**
